Question
抗血小板薬を服用していますが、抜歯するときは薬をやめないといけないのですか？
Answer
これまで、ワルファリンや抗血小板剤などを服用している患者の抜歯について多くの研究が行われています。いずれの研究も抜歯に際してそれらの薬剤の服用を中止しないことを推奨しています1-10)。
抗血液凝固剤を中断したために生じる血栓問題のリスクは、中断しないために生じる出血のリスクを上回ることが報告されています11)。
抗血液凝固剤を服用している患者の歯科外科手術後に局所的止血法以上の手段が要求された症例は、極めて稀であることを示す論文もあります12-13)。
950人の患者に対する2,400の歯科外科手術のうち12人の患者(1.3%以下)だけが、局所的止血法以上の手段が要求されたとのことです11)。
また、抗血液凝固剤服用患者で歯科外科手術後に入院したり輸血を受けた患者は非常に稀であるという報告もあります14)。
その他の多くの論文も、抗血小板剤を服用している患者の歯科外科分野での出血は、局所的止血処置で処理できることを示しています15-19)。
他方、歯科外科手術のために抗血液凝固剤の服用を止めたり減量したりすると、致命的な血栓症が生じることが時々あります。
抜歯のためにワルファリンを中止した場合、高度の血栓症の発生率は0.95%であり、その場合大多数の患者が死亡したことを報告しています12)。
ワルファリンを減量したり服用を中止したりして行った2,673人の患者の2,775回の歯科外科手術のうち、血栓問題が22回(0.8%)生じ、そのうち6人(0.2%)が死亡したことを報告しています11)。
そのほか歯科外科手術症例ではありませんが、ワルファリンの使用を中止したり減量させたりした後に内視鏡検査を行うと高度の血栓症が1.2%発生し、その場合大多数の患者が死亡するか不具になることが報告されています20)。
